Privacy of our patient’s health information is an important principle to Calgary Vision Centre. We are committed and accountable to collect, use, disclose and protect a patient’s health information in a confidential, secure and appropriate manner.

Our clinic collects, protects, uses, discloses, allows access, corrects, amends and disposes of health information in accordance with the Health Information Act (HIA).

The health information that we are collecting is needed to:

  • Provide you with diagnostic, treatment and care services

  • Provide for the training of students. (if required)

  • Provide for research or statistical purposes. (if required)

  • Provide for other authorized purposes(s) under Section 27 of the Health Information Act. (if required)

  • Add/remove additional uses as relevant to your clinic

It is collected under the authority of section 20(b) of the Health Information Act, as it is directly related to and necessary to carry out an authorized purpose under Section 27.

The confidentiality of this health information and your privacy are protected by the provisions of the Health Information Act.
